PHQ Question.
« on: Sep 24, 2014, 11:04 »
Howdy guys.  I'm working on filling out my PHQ to go up to Wisconsin next week, and it asks for the date that my last UA was favorably terminated.  My question is, do I need to call PPL Susquahanna where my last UA was to have the exact date? Or can I just put a general date which would have been around the middle of June sometime?

As a second question instead of calling PPL Susquahanna, is there a sight that the NRC might have that I can find out when my last UA was terminated?

Thanks for any help.

You can put the date that you were laid off down.  Usually, when you get to the site, it has to be reviewed and they have the date that the last place you worked gave them.  The two dates will only vary by a few days, depends on when security actually termed your badge.
It is what it is!

You should call your employer for the date they laid you off.

I recommend that every Road Tech keep a good record of where they've worked, who they've worked for, etc.  Because of the information Commonwealth Edison always seemed to want (back in the '70's), I started keeping that list and it's come in handy over and over again.

Who you worked for - Company, office address and phone number
Where you worked - the site's street address and phone number.  Include your immediate supervisor and/or site coordinator here.
    Date you reported on site
    Date you left site
Where you stayed while working at that site (motel, rental, etc.)

It's easy to set up and even easier to maintain.  You don't even need a computer.

I have a spreadsheet with similar information going back to my 18th birthday. Very handy.

Oh my . . . I don't keep track of how much I worked . . . only how long I was employed.

Correction noted.  When I was employed by ARC, I was a deconner, so I did indeed work.
