HPIS maintained by the NRC. Is HPI an effective tool for monitoring site performance? How is information collected? Out of all metrics possible to monitor,what percentage do you think gets recorded vs. not recorded? How valid are the statistics?
Also are error precursors discussed in preplanned or other meetings? If so does this have any meaning full impact?

HPI is all over the country in a multitude of industries.Formalised that is. What is your experience?

HPI still exists.Just wanted to see a new date.Could the error precursor for this topics responce or lack there of be apathy?
