"U.S. nuclear facilities lack the "best-in-class" preparedness necessary to ensure government workers and the public would be protected in an emergency, the Department of Energy's inspector general said in a report released this week."

Is this the DOE equivalent of an INPO 4?

The Defense Nuclear Facilities Safety Board is as close to an NRC equivalent you will get. It's mission is similar but you don't have Generals, Admirals, and National Nuclear Security Adminstration officials involved in NRC processes making it a very big game changer.
