During the last twenty years I have requested copies of my site physicals to be forwarded to my doctor's office. Imagine my surprise this week, when I was reviewing my file and not a single one was in there. I queried the personnel but they said that if they had received any physical examination information requested to be placed in a file, then they would have done so. My question at this time would be primarily to nuclear power plant personnel: who would I contact to retrieve this type of information from such a long time ago?

release of the information is now a legal issue and not medical.. send a notarized document requesting release of information to each HR department requesting copies of your file. It will take weeks.

Thanks for all the help. Vikingfan, I appreciate that link. However, I am a bit embarrassed that I didn't think to use the nucleartourist.com myself. I guess I should always think first and ask second. :-[

Note that that is the utilities address's, not the plant address's. But for medical records, it should work. Just don't drive there, and expect to see steam.
