Title: Feds: Firm dropped part for SC nuclear plant, covered it up

ATLANTA (AP) — Federal regulators proposed an $11,200 fine Tuesday against Chicago Bridge & Iron Co. because its workers dropped a prefabricated section for a new nuclear power plant in South Carolina and then tried to cover it up, officials said.

   It would seem that the rigging manager and safety representative are culpable for individual enforcement as they directed subordinates to falsify documentation.

If you read the announcement, they also talk about action towards a former employee.

Care to take any guesses?  ;)

On a serious note, this actually happened in 2013.  the sub-module in question has been shipped here, inspected, found to have no defects from the drop, welded to other sub-modules and the entire module is now in its final, installed location.
