I'd like to include a few pieces of information from my own personal findings.  Concerning formatting; understand that placing your thoughts into a well formatted document is of great value.   The easier it is to follow, and understand – the better your chances of obtaining said waiver.  Covering the “Who, What, When, Where, Why, (and How)” is listed atop the hand written page; do it.  It may be valuable to do it in thesis format – however keep it as short as possible.  Furthermore, please remember that the page your writing will NOT be handed to the concerned authority as an original.  Your waiver form is going to be faxed – perhaps SEVERAL times.  Write large, clearly, and in LEGIBLE handwriting.  Reading a collection of well formatted thoughts, with correct punctuation, grammar, and spelling will be contributing factors to a successful application.  Spacing your letters far apart, and writing in large text may seem a bit “kindergarten-like” at first.  It will be appreciated however – and may save you the trouble of rewriting your waiver application.  It would be wise to type it first to ensure proper formatting, spelling, and progression (putting like thoughts together).

Remember that your applying for a job; furthermore you are detailing the flaws in your character; of past, and present.  (Hopefully not?)  The easier you make it to accept you, the more likely you will be accepted.
