Pellets found everywhere. Some of the old Westinghouse workers shot defective fuel pellets at passing trains, allegedly. :-X
Numerous NRC visits, loss of radmat control issues, schedule issues solved by RIFing HP (finding too many issues), etc.  :-X
March 5, 2014 NRC conference call alluded to Final Status Survey shortcuts that would make "reviews of the FSS documents will be long and tedious." and "demonstrated a lack of quality and quality control." http://pbadupws.nrc.gov/docs/ML1407/ML14071A254.pdf
