OCS Paperwork for Submarine Officer

So I am working on an application for OCS and one of the forms I'm using is the Fleet Officer Application Checklist (NAVCRUIT 1131 56).  This form tells me that for individuals applying to be a Nuclear Trained Officer they are required to have a "Nuclear Power Drug/Security Questionnaire" form filled out.  I am unable to find this form and have consulted with the instruction (OPNAV 1420.1b), my Command's CCC and the Squadron CCC with no luck.  I have sent an email to the OCS help desk, but figured I would see if anyone here knew where to find it off the top of their head.  Thanks.
