An error like this might be embarrassing at most sites but stormwater drains at Y-12 have a different importance.

Construction on Y-12 mercury treatment plant could start in 2018, cost $146 million

Large quantities were used during the Cold War for nuclear weapons research and development from 1950 to 1963.
An estimated 24 million pounds was brought to Y-12.
More than two million pounds was spilled or lost, or officials haven’t been able to account for it.
About 700,000 pounds was lost to the environment, including:
Contamination in process buildings and soils—428,000 pounds
Releases to Upper East Fork Poplar Creek—239,000 pounds
Contamination in New Hope Pond sentiment—15,000 pounds
Airborne releases—51,000 pounds
That leaves about 1.3 million pounds that officials haven’t been able to account for.

...Y-12’s managing contractor convened a meeting of the Operational Safety Board to authorize emergency work “to allow expedited modifications to the storm drain and stabilize the construction site in advance of pending storms.”

ya know, about 40 years ago or more the PM would have walked over at 4PM, surveyed the gaping hole of a mistake and then turned to the General foremen and rebuked, "I want that dam thing fixed before I get into the office tomorrow morning!!!!"

and,..

it would have been fixed,...

and the cumulative salary costs of a dozen members of the OSB authorizing emergency work would have never been spent,...

sometimes people wonder why everything costs so much and takes so long,...

...Even though the UPF project office has a site representative to ensure “configuration control” of Y-12 systems is maintained during work on the project, it took several days for the error to be recognized and several more days for the Corps of Engineers to inform the Y-12 contractor and the UPF project office....

I do not wonder,...

I had a PM tell me that he works a DOE project once in a while to remind him why he Does not work DOE
